Victims of crime, family members of a victim or witnesses who testified against an individual in state custody may register for notification of parole hearings, the scheduled release or the escape of the individual by completing the California Department of Corrections and Rehabilitation (CDCR) Request for Victim Services Form 1707. You may either download the CDCR 1707 form and send it to the CDCR’s Office of Victim and Survivor Rights and Services or apply online by visiting https://www.cdcr.ca.gov/victim-services/application/.

Be sure to keep your contact information current with CDCR to ensure that you receive notifications. Personal information about victims, including addresses and phone numbers, is confidential and is prohibited under California law from being made available to the offender.

Victim services representatives with the District Attorney’s Office are available to assist crime victims and their relatives in completing the form. To reach the Los Angeles County District Attorney’s Office Bureau of Victim Services, call (800) 380-3811.
